The sequence

How tolerance is lost, step by step

Autoimmunity rarely begins the day symptoms do. Most conditions have a silent preclinical phase measurable in blood years earlier.

1. Healthy defence

Established evidence

A pathogen enters. Macrophages engulf it, present antigen, and T and B cells mount a targeted response. Regulatory cells shut the response down once the threat is cleared.

2. Tolerance is tested

Established evidence

Genetic susceptibility (often HLA-linked) combines with environmental triggers — infection, smoking, certain exposures, and the microbiome — to prime autoreactive cells.

3. The mistake

Emerging evidence

A self-protein resembles a foreign one, or damaged tissue exposes hidden antigens. Autoreactive T and B cells expand and autoantibodies appear, sometimes years before symptoms.

4. Chronic attack

Established evidence

Inflammation becomes self-sustaining. Tissue is damaged, more self-antigen is released, and the cycle repeats — producing flares, remissions, and organ damage over time.
