Diseases with a strong Middle East connection
Behçet's disease
Behçet's disease — recurrent oral and genital ulcers with eye, joint, and vessel inflammation — is markedly more prevalent in countries along the ancient Silk Road, including Türkiye and the wider Middle East, than in Northern Europe or the United States. HLA-B51 is the strongest known genetic association.
Familial Mediterranean Fever (FMF)
FMF is an inherited autoinflammatory disease causing recurrent fever and serositis, most common among Turkish, Arab, Armenian, and Sephardi Jewish populations. Consanguinity in parts of the region increases the chance of inheriting MEFV gene variants, and colchicine remains the mainstay of treatment.
Multiple sclerosis in Arab populations
Once described as rare in Arab countries, multiple sclerosis prevalence has risen substantially across the Eastern Mediterranean. Improved MRI access, better case-finding, and dedicated national registries explain part of the increase, and risk-factor studies are now under way in regional cohorts.
Lupus in Arab women
Cohort studies from Saudi Arabia and other Arab countries report lupus nephritis and severe organ involvement at diagnosis more often than in many Western cohorts. Registry sizes remain limited, so these findings describe treated populations rather than the full burden of disease.
Kikuchi-Fujimoto disease (KFD)
KFD is a rare, usually self-limiting inflammation of the lymph nodes that most often affects young women and can mimic lupus or lymphoma. It was first described in Japan and is increasingly reported across the Middle East, including Saudi Arabia — the disease the AN's founder was diagnosed with in 2024.
Why the region looks different under the microscope
Vitamin D in the region
Vitamin D deficiency is widespread across the Eastern Mediterranean despite abundant sunshine, driven by reduced sun exposure and other factors. Low vitamin D is associated with autoimmune risk in observational studies, but supplementation has not been proven to prevent autoimmune disease.
Genetics & consanguinity
Consanguineous marriage, common in parts of the region, increases the visibility of rare monogenic autoinflammatory and immune-dysregulation syndromes. This makes Middle Eastern populations especially valuable for gene discovery — several immune genes were first identified in regional families.
The data gap
Most global autoimmune prevalence figures come from Europe, North America, and East Asia. Much of the Middle East and North Africa remains under-represented in large registries, which means prevalence in the region is genuinely uncertain — and a priority for new research.
